We recently moved back to Canada and are applying for new certificates as their last names have recently been changed. I'm trying to complete CIT 0001. Do I still need to complete the section for the "other parent" (the Thai surrogate)? I do have a waiver that the surrogate signed to waive their parental rights and we submitted that along with our application and left the "Other parent" part blank. Will I need to submit that waiver again with my application?
