Missing name on PR Card

Hi,
My wife received her PR Card last week and she is missing a name on the card.  For example, she goes by First Middle1 Middle2 Surname.  But the card lists her name as Middle1 Middle2 Surname.  She immigrated from the U.S. and now we are finding difficulties in registering the car she brought over and getting her drivers licence because the name on her PR Card does not match the name on her car title and on her US drivers licence.  Has anyone encountered this before?  What do we do?  She doesn't want to register as a completely different person in Canada.
 
This is very frustrating as it took 3.5 years to get to this stage (PR application, application rejection, appeal, ADR, two TRPs -- and now a missing name on the PR Card).  Any advice would be greatly appreciated.

What does her name look like in her Confirmation of Permanent Residence form? If it's the same with the PR card then it seems that when CIC asked to check her Visa if there were some errors before she landed, she did not inform CIC...

However if that was the case (name is different in PR card) the questions is how did Immigration accept her as the same person on her passport?
 
If the name on her PR card is actually different from her Passport and/or CoPR, then she needs to call the CIC Call Centre, explain the error, and she needs to ask them how to get hold of a form to reissue the PR card.
