Statutory Declarations from friends and family

I'm in the process of getting all my documents together for my PR visa.
 
Do the Statutory Declarations from friends and family to prove our relationship need to be in a certain format? ie I do solemnly declare
 
Do they need to be signed  by a Commissioner of Oaths?

Are you applying common law or spouse? You don't need it for spouse.

If you are applying common law, I can only answer the certified part, which is no, they just need to be signed by the person it is from :)

it is for common law so they are just to prove we are in an ongoing common-law relationship.

actually depending on where you are applying from....there is a list on the forms which state which countries require notarized declarations.
