Could somebody kindly tell me how can somebody get the health record from OHIP in Ontario, I remember reading that you have to send a letter in Kitchener probably? Anybody has any details please and thank you!

Hi RC, you can try contacting Service Ontario at 1-866-532-3161 (Monday to Friday 8:30 am to 5:00 pm). They should be able to guide you on how to go about obtaining your health record.

Hi,
You can request a Personal Claim History by sending a letter to the following address:

Ministry of Health and Long-Term Care
Personal Health Information Office
Corporate Administration Services
49 Place d’Armes 4th Floor
P.O. Box 48 Kingston, ON K7L 5J3

The request must have your name, health card number, date of birth, current address, and the time frame.
You will receive the requested information in about 10 calendar days.

Thank you  both of you.  TNGC, is there a webpage where they have directions on how to do this and should one send the copy of their health card or just write the number in the letter? Thank you!

You do not have to attach a copy of your health card. The health card number is already part of the requested info. You can write something along these lines:

Ministry of Health and Long-Term Care
Personal Health Information Office
Corporate Administration Services
49 Place d’Armes 4th Floor
P.O. Box 48 Kingston, ON K7L 5J3

Dear Sir / Madam,
I would like to request a Personal Claim History for the period of .... 20xx to present.

Name:
Health Card #
DOB:
Time frame of history: 20xx to present
Mailing and physical address:
