I am attaching these documents... form itself..
1. Photocopy of my entire Passport

2. Photocopy of Birth Certificate

3. Photocopy of Study Permit and Student Visa

4. Photocopy of acceptance from Humber Institute of Technology and Advanced Learning

5. Photocopy of Bank Statements, Income Tax Returns and Sponsorship Letter.

6. Photocopy of transcripts    --- as i am in india now and i have attended the course last time so do i need to attach photocopies of my school and college transcripts?

yes you need to show proof of funds when you do an extension..paid amount for the term and proof of funds for your expenses. I showed around CAD 5500...CAD3000 receipt from my university and CAD2500 in my bank....and your transcript from your Canadian university/college...You DON'T need birth certificate and acceptance letter.

If you want to extend student visa then you have to meet the immigration agent or you can see the immigration site of Canada because with that you can get all the information.
