2-part question for criminal rehab form

We are filling up my husband's criminal rehabilitation forms and on the part where it says list all addresses since you were 18, my hubby  can't remember exactly what street he lived on in the USA. He was mainly 'couch-surfing' for a while, so he didn't have a permanent address. Should we 'make one up' or what? On the form it states that if an address is not listed the form will be returned.
Also, we cannot say that he never lived in USA since that's where he was arrested. (He's not a usa citizen, he is mexican and was living illegaly in the states).

Part 2 is that we had originally wanted to get a visitor visa, but from my understanding we need to do the rehab part first....Do we send the rehabilitation forms and also application for Visa in the same time? Or how does it work? thanks!
