affidavit for chinese stamp in passport

hi guys
need to get my pr card renewed. I have a chinese stamp in my passport. does anyone know the format of writing the affidavit for that stamp to be converted into english.
and also they are asking for a certified copy of the original document. now what is this original document. the affidavit or the passport.
any help will be appreciated

Usually it means that the notary certified the translation as authentic, alongwith the copy of the doc being translated.

I don't think you can do it yourself, unless you're a notary....

Actually the OP wants a certified translator who certifies that his/her translation is correct, not a notary, who certifies that a copy of a document is the same as the original.
