Off campus work permit Problem

Hi, 
   I have fulfilled all the requirements of OCWP but when i'm trying to enter the details It pop-ups that we cannot match the details provided even though the details I have entered are correct as per the document provided by the CIC. 

Hope the seniors will help me with their guidance as whom to report or contact. Thank you.
