Does CIC Toronto carry supplementary IMM 5455?

Hi Everyone,
I need an IMM 5455 form to apply for renewal of my PR Card. I have an emergency so I can't wait for them to ship it to me. Do you know if any of the CIC or service Canada office carry this form? I need to go this tomorrow early morning.

Please write the address as well if you know.

Your help is much appreciated.
