Urgent Guidance on Documents to send to CIO

I have been nominated for immigration to Canada by Alberta province. However prior to this I had applied for immigration and my application was approved but could not immigrate within the 3 year in 5 year allowable period. I have already taken steps to relinquish my previous status so that I can reapply.  Should I include details of my relinquishment of previous status while sending my application package to CIO?
