Hi,

 I am sponsoring my parents.

 Does my Dad only has to fill this form "Additional Family Information"??? My Dad is the principal applicant

 Should my Mom also have to fill a separate form???

Are they Applying for a super Visa or your are sponsoring them?? CIC nolonger take Sponsorship Applications for Parents and grand parents are you aware????

I am sponsoring them. I have sponsored them long back in 2008. CIC want this form to be filled and send it to them.
 
I am just confused about this form. Does both my parents have to fill this form??

Yes. Look at the form, they list parents, brothers sister, I assume that they don't have the same parents or brothers and sisters.

I guess this form is required to be filled for all over age of 18 including principle applicant and dependants
